There are several ways to enroll

1. Go to BC-HHS. Take your ID and proof of address. If you are homeless, you may use the address of Longmont's HOPE shelter offices at 804 Lincoln St, Longmont, Colorado, 80501. This is the safest way to enroll.
2. Use the PEAK on-line enrollment method here. You will need to create a PEAK account if you don't already have one.
3. Fill out the paper enrollment form and mail it to Boulder - Department of Housing and Human Services P.O. Box 471 Boulder, CO 80306. Paper forms are available at many support services. You can print one at the Longmont Library. This is the slowest way to enroll and it is not easy.

If you enroll in 2027 or December 2026, be ready to prove that you met the work requirements in the month before your coverage starts. A case worker can tell you if you may claim an exemption instead.
